Try pathologic. Make sure to get the original, word salad version for the foreigner feel.
Also try S.T.A.L.K.E.R Soc or CS with the Russian voices (unless you are Russian). No other game has gotten close to giving me the feeling of being alone in another country like my first playthrough of S.T.A.L.K.E.R SoC.

I wonder how STALKER gameplay would be affected by a megacity setting. It would certainly flow differently when all the environments are so dense and massive. You could spend an entire day just explore one realistically-sized skyscraper. Maybe a procedural survival game with realistic bullet physics would suit the setting better. I look at some of these skyscrapers and imagine a firefight breaking out between warring gangs in adjacent buildings, then climbing up to ambush a wounded party and steal their stuff.

I think Fragile Dreams does something like that, with the premise of a world after most of humanity has vanished, and the protagonist left to wander the decaying ruins after the old man taking care of him dies prior to the opening, exploring with a battery powered flash light, resting next to trash fires, and fighting lingering regrets of the dead.
