The Box art is usually designed by the marketing team and picked by committee. They have committees of people who determine "x amount of people prefer this box". The box art also represents your game far more to investors of your company. There's so many fingers in the box art pie that they eventually come up with something bland and inoffensive to appeal to as many people as possible.
This was most transparent with the game Bioshock Infinite where they admitted the cover of the game was designed after they went to "frat houses" and asked them what games they played and designed the cover to appeal to that demographic. The cover to this game is so dumb and really betrays the actual attempts at genuine art direction the devs put into it. Another example is Ico where the Japanese and European box art looked genuinely artistic but the US box art looked bland as hell and was stated as one of the reasons why the game sold poorly in the US.
It's only in Japan you see really nice looking box art generally now. And it's because over there consumers are different and respond more to nice looking boxes rather than generic ones.

This is incredibly good looking, because one wonders what kind of world this is, and leads to a lot of questions and further desire to see more of it, as compared to pics 1 and 3 which denotes a story that is character driven rather than event driven, or a world that molds itself to its actors. Call it the cancer of humanitarianism that states that all have an inherent value and so are worthy of highlighting, rather than the reality which states that an iron ingot is of no use until it is forged and tempered by hammer and heat, the "events."
That's my philosophical take on the matter. In fewer words, modern box art reflects a giant case of ego.

MMOs were basically a meme genre. People loved the idea of being part of a constant ongoing world in a game but eventually found out that most games did jack to actually take advantage of this premise while having all sorts of problems that were easily bypassed by just not playing an MMO.
That's kind of part of why Battle Royale got popular, it took the sort of shit people wanted in an MMO while taking to actual MMO part out.
