I've been mulling over a homebrew on and off for a few years now

>society of eunuchs: if you're male, get your family jewels lopped off for the following trade-offs: lose the manly bonuses to physical stats and gain resistance to social influences, especially from females. Incur penalties when attempting to seduce females. Gain bonus when attempting to seduce males, if you're playing at a table full of degenerates, I guess Crotch shots that successfully land are no longer debilitating. You're all sewn up down there, so you gotta squat/sit down when you pee!
>society of barrens: if you're female, get your ovaries torn out for the following trade-offs: small bonus to physical stats, but lose some of your bonuses for social stats. Congrats, little boy, you grew your first mustache! Incur penalties when attempting to seduce males. successful attempts at seducing females results in them feeling cheated and angry with you when they see you can't give them the dicking you made them want, if that's the sort of degeneracy your group is into There's the option of going the whole hog and getting the double mastectomy, which lets you gain full bonuses to physical stats, and you lose the rest of your social bonuses. Blows to the chest that land successfully are no longer debilitating. You also have scar tissue in all appropriate places.

>last note: you have to roll to see if you survive the ritual. Both societies tout "do it to be free of the biological realities that limit you!" but risk of death is very real with any surgery, and even more so with the tools of the setting
The half-way genderswap isn't even the main draw of the game I've been mulling over; it's just an afterthought I had after playing age of wushu for a bit. The main draw is supposed to be martial arts with some elements mixed in. I haven't decided if it should be down to earth low fantasy, or over the fucking top, like jumping up to rooftops, running on water, and snatching flies with chopsticks; or if the elemental stuff is just adopting its aspects into your fighting style, or going full-on Avatar with it
