I can't think of a single game made in the past decade that fits this description. Every single game I can think of is ultimately either some derivative low-risk bullshit designed to pander to a specific audience OR a proof-of-concept indie game (typically a puzzle game) that lasts for all of two hours even if you hunt for every damn easter egg in the game.

No, but most of the good ones have been. You can still make a 95% unique, original game, but it's probably going to suck. It's frankly a better use of effort to improve upon/perfect decent ideas that have been used in the past, but not to their full potential. 9 times out of ten I'd rather play a great game based on old ideas than a shit game that's totally new.

Creeper World 3, Particle Fleet, EDF4/5, Deception 4, Factorio, every Zachtronics game. You could argue that sequels don't count, but I would argue that they do as they are sequels in a series that is still completely unchallenged. It is also very easy for a sequel to fuck everything up and be a completely different game; see Command and Conquer. A faithful sequel is still just as much a miracle as any new game that is fun, interesting, and unique. I will be glad if anyone can prove me wrong as I would like more games to play.

It's kind of funny, the first Nier was certainly interesting and unique to me, but it wasn't that fun (compared to katamari, at least). Now that I'm playing Automata, it's very fun as the gameplay is much better, but as of ending A, I'm still not very interested in the plot or world in general (maybe it gets better, but there's a ton of unanswered stuff at the moment).

Everything is iterative. In a healthy world we would see iterative design improvements, evolutions and the slow additions of better gameplay elements where they fit. Our world however is more akin to a 'race to the bottom', one success story causes tons of worse clones to milk it to death, then the locusts move on to find the next cash cow.
I do admit that I really liked how Titanfall 2 functioned in its singleplayer.

Both Gravity Rush games are great, but the second does do a lot to expand on the gameplay and customization while directly incorporating more ideas involving gravity (even the movesets start reflecting it when you get the alternative styles). Is a bit annoying that some minor content for GR2 will soon be permanently lost with the servers though.

Not "hurr durr every platinum game is equal" like these retards, but what it did was simply polish the original Nier's combat and present the story in a better than the the original I did like the text segment, but it was quite obvious that it was only there because lack of resources. It was interesting and fun though, so I'll give you that.
I guess the original Nier does fit though, since I think the gameplay was different from Drakengard's but I don't know since I haven't even beaten the first thanks to fucking PS2 emulators not working.

I definitely enjoyed it when I played it. Lots of cool things you can do in the game. You can destroy bullets with other bullets, even cut bullets with the Katana. In one of the special modes time would freeze instead of slow down, letting you dodge bullets like neo. Your body's profile was far larger sideways. So if you played your moves right you could ever so slowly move your way towards an enemy with a rifle as long as you continued to dodge every shot.
