I'd recommend just playing any of the games that GR3 mentioned as favorite games from their childhood, the in-game ROM list is a good reference.

The games explicitly referenced at the beginning of the manual are The Maze of Galious, The Tower of Druaga, and Hydlide II: Shrine of Darkness. I'd also recommend King's Valley, The Treasure of Usas, and the MSX variation of Akumajou Dracula. If you're willing to try something that isn't an archaeological exploration adventure you might also check out their first game, GR3, I'd recommend the ReDeath variation.

What does it takes to appreciate games like this?

I thought only people who had an MSX would enjoy this.

I tried the pretty remake of this and made some advaces (defeated three bosses) but had to some some guides to advance and realized I wasn't enjoying the experience.

Can't think of anything similar I've played, VVVVVV, Cave Story, Shovel Knight and even the ancient Revenge of Montezuma look nothing alike in actual gameplay.

Environmental Station Alpha is completely different in theme and setting, but it has the whole mystery and puzzle solving atmosphere that La-Mulana does. However, there's an entire initial portion that's basically a whole standard Metroid-style game, and you have to look way deeper to get to the weird shit. I'm sure there are people who have beaten the "final boss" without ever knowing there was more to the game.

Growing up with classic games on a classic console/home computer certainly might make you more inclined to like this sort of game but I dont think that's really a requirement.

It seems like they're putting more thought into the backstory this time instead of making it up as they go along, so there's surely going to be some explanation for it. Hopefully the explanation isn't that Lemeza failed to destroy her soul or something along those lines.

A large amount of patience and a small amount of masochism. Keep in mind that a blind first playthrough could take you 40-60 hours, so it's a pretty fucking big game relative to other games in the "genre". You may have been making progress at a reasonable pace even when it didn't feel like you were. Either way it's a niche interest, they never expected the game to catch on as much as it did so you shouldn't worry if it doesn't appeal to you, it doesn't appeal to a lot of people.

If you ever feel compelled to go back and try it again sometime I'd recommend playing the original, in my opinion it's a little easier than the remake and it's a more purist MSX-style experience as well.

Something always bothered me with the remake. I could never advance past a certain point before I lucked out and got a key fairy to open the way to the Gate of Time, then I'd use the secret entrance to get deeper into the Tower of the Goddess and then it would be smooth sailing from there.

The trigger for the Tower of Goddess to appear normally is defeating Viy and returning to the Surface. This needs 2 or 3 seals, which needs Bahamut defeated as well.

It's a doujin game most likely made by a single person, unsurprising that it would be kept visually simple to limit the amount of time spent creating assets. Your first playthrough can be done in one sitting, go give it a shot. There's an english patch available as well.
