I'd go with the "sexy underages" idea, but with deep mechanics and well-developed characters. You'd play an adult male fighter pilot dating a teenage girl who's redpilled about the press and the schools lying to her, with the adults who oppose their relationship depicted as either dupes of the media or jealous, petty hypocrites. In other words, I'd tell the truth. You'd fly a special prototype fighter and do black ops at first, then huge battles later. The pilot may be forced to choose whether his society is even worth saving, and the plot could include alternate paths where you side with an enemy faction or strike out on your own. You'd have wingmen, and you'd give them orders and customize their vehicles. The plot would have Earth being part of a space proxy war between aliens and robots.
The political situation highlights the dehumanization that people hated by society endure, and the fake reformers who perpetuate it. Earth is a totalitarian shithole that nominally governs itself, but is under the thumb of greater powers. The idea is that humans talk about freedom while denying it to others on the basis of stupid bullshit – so pretty much what assholes do when they're not in power. You'd delve into alien culture and find that Earth's government made up a watered-down, distorted take on it as a form of propaganda and to subvert alien society. This jabs at how shallow liberal storytelling is, and how they twist things to fit their narrative. The robots are much like the Cylons from the 2000s Battlestar Galactica with some NieR Automata thrown in. A scientist programmed them to fuck up the galaxy, but some of them gain individuality. Basically, the conflict is between a group that murders people because of their dumb beliefs, a group that does it just because they were told to, and a group that does it for personal gain. Any more detail would require actually making the game, but no store would carry it, so it's wasted effort.

I was thinking about something like a "VR simulator" of the holocaust. Model all six death camps with great accuracy (and good graphics = normalfag bait), then make the game a simulator where you run all six camps (the only ahistorical aspect of the entire game) and have to take just what resources you're given by higher command and manage the camps.
Players who actually start executing prisoners and try to make the holocaust happen are almost instantly met with both riots and mass escapes. A week later (in-game) high command sends representatives to execute you (and any of your staff who didn't refuse your orders) for mistreatment of prisoners and crimes against humanity. In that week, you'll also notice that each of your crematoriums can't incinerate more than one body every two hours. Once you restart and actually manage your camps like the Nazis actually did, you start running out of supplies and people start dying of starvation (which makes the player feel guilty) and they see the crematorium restrictions again anyway. Eventually Heinrich Himmler sends you down a message "The death rate at the camps ABSOLUTELY MUST BE REDUCED" and you can even show the player the real-world letter he sent to camp commanders.
Get a couple of physicists and historians to put their names to quotes (vague things like "A scientifically accurate representation of the horrors of the time." and "Go inside the camps of the Nazi regime.") and you'll actually bait some hard bluepills to play it.

Third person wrpg/action game.
You play as a BBC-style XVIIst century slave trader. The game includes traveling by sea, buying and selling, logistics and "combat" that consist in whipping captured slaves or unruly crew members, occasionally executing some of them.
The mc is the captain of a ship, accompanied by his wife (kids only mentioned, back in Europe). A couple of action segments should include beating her unconscious. Tribal niggers freshly taken from Africa would of course as vile and animalistic as they were (are) in real life : routinely fighting each other for food, trying to mate with females struggling with their restraints, ape-like begging behaviour while interacting with whites, pretty much always asking for food. There should be a quest line where one or several niggers screw over the rest of the slaves for a puny advantage.
Life on the ship should be lively, with people working, making fun of niggers, chatting, etc. Edgelord version of that one pirate Assassin's creed game basically. Pretty much every single character is scum, and the main character should be the worst of them all. Except one Dutch sailor with a mustache, calling him Adolf might be a little too obvious but you get the idea. Even the wife is a hysterical, badmouthing cunt who relieves her nerves on anyone around by throwing tantrums.
While selling the goods in Virginia, it suddenly becomes apparent that the main character is Jewish. This could be done by talking with his peers in Hebrew or Yiddish, him casually walking into a synagogue (not sure if there were any in America at the time), or a random not-an-asshole character pointing out that it is very unchristian to treat his crew so badly, to which he answers … and so on.

It's a first person shooter. In the year 2067 you were once a lawyer for a corrupt politician. After discovering her corruption (the senator looks like Hillary Clinton) you attempt to go to the authorities but you are arrested for the brutal murder of some 15 yr old girl who was supposedly boffing your boyfriend. The prison isn't exactly the nightmare you heard of in the news and from rumors. Things are kind of weird though in the fact that a number of men who are apparently pregnant are walking around, as well as pregnant women. Women in the prison either fight to keep from being raped or end up being whores for the male prisoners to survive. You actually have the option of fighting against being raped or going along with being someone's whore and there are plenty of sex scenes. There are also plenty of scenarios where you can choose to have sex with prisoners or rape and/murder prisoners in various ways.
After being into the game for more then a hour you will be sent a message on your explosive collar that your collar has been deactivated and that you and few others are going to be given the chance to escape because all hell is going to break loose. The said hell that breaks loose is a sudden prison riot due to zombies and other weird monstrosities appearing out of nowhere. Some prisoners will also turn in to rape happy monsters or whatever. You will manage to get a prison guards gun and you, a child rapist, a pregnant black woman (who is the big whore in the prison who loves anal), a gay dude who is now pregnant, and a tranny/hermaphrodite escape out of the prison onto the island.
You discover that the rest of the island isn't the nightmare you heard about, its actually a perverted naturists paradise. Incest, pedophilia, and kiddy porn are totally legal on the island and the island is basically run by a number of corrupt corporations and a few international politicians. You eventually meet up with said boyfriend and find out he's a pedo with a incestuous backstory and that he was somehow framed and put on the island as well.
The main idea of the story will be you figuring out how to survive on the island, discover the cause of the monster outbreak, and maybe discovering a way to prove your innocence. But you will have the choice of choosing certain factions that you can work with. You can choose some rebel group, become a servant of some corporate asshole, sign up with a bunch of pervs who want to take over the island, or even some communists. There are a number of small and large factions on the island and you can choose who to work with, work against, and betray at a moments notice.
The missions involve basically killing monsters and people, as well as some crazy stuff such as blowing up a abortion clinic. The choices you make have a effect on how you play the game and how people you deal with react to you. Such as if you were a whore in prison you may end up pregnant from all your whoring around and have to play the game while your belly slowly gets bigger and bigger. Blowing up the abortion clinic will increase the number of humanoid monsters on the island, but will may decrease the number of other weird monstrosities you have to fight. The choices you make can greatly affect your gameplay.
Weapons include a dildo and onahole gun, chainsaw and various guns ranging from standard to sci-fi. The game will have plenty of racially offensive stereotypes and offensive and often racist language. And finally you get to choose between three characters to play as, a big tittied blonde slut, a horny, but average gay man, and full hermaphrodite with a big dick and humongous tits.

To be fair, you don't have to be subtle but simply careful with timing. Start out with a nigger in your party that is 'coolguy' and a kike doing bookkeeping. After you've played a good halfway through the game the nigger can't solve a simple 5x2 question and chimps out and if you don't keep a close eye to your inventory the kike simply steals your gold and valuable loot. That last part can be done without ever accusing the kike, though when you kill him the missing gold can be found on his corpse along with the gold value of the stolen loot. Also the missing gold/loot issue stops happening.
Also if you add difficulty to the game most reviewers won't reach the halfway point so they will not notice this offense and your game will still sell. I guess if we need to able to offend ourselves the MC should be a trans faggot that has to sick the niggers cock to progress through the game.

Protag would be a german wehrmacht soldier, during 1944. The game goal is to escape from Berlin while you would witness the savagery of the Red Army. At the final bonus stage you would be playing after the war as an old man in Brazil, while you need to defend yourself and your escaping family members from jew nazi hunters. You start with basic email attachment viruses and nigerian prince scams for some low ranking Israeli government official wanting extra shekels.
Then you'd start tracking down and niggering wanted individuals, at first in India, then Brazil and finally the US and Europe etc.
You'd first have to discover the guy and then observe him for certain traits your employer wants(likes/dislikes, habits etc.) and in later stages use these to blackmail, kill or otherwise eliminate the target through various means.
As you and your botnet grow in size you'd eventually get on to spying on foreign and domestic diplomats, faciliating illegal weapons trade, manipulating stock markets and inserting hardware botnets into consumer products at certain stores to root out suspected GNU/terrorists.
All the time you'd have to fend off other letter agencies from buying zero-days from Microkike first, snuff out whistleblowers and avoid getting botnetted yourself by foreign government agencies.
The final mission would be arranging the transport of a high value prisoner on a plane somewhere without the plane disappearing or crashing.

I'm not sure if it would be the most offensive to me personally but it would probably be some sort of game that advertises itself as extremely inclusive and politically correct. All of the developers and press releases for it reassure that it is going to be a non-triggering experience. There would be characters from like every single diversity quota possible and have them all be playable characters or party members. The game would need to be heavily accessible to even the most casual of people so it would need minimum gameplay/mechanics, colorful and cutesy characters, simple to follow story line, etc. However there will be one non-inclusive character, a straight male. Throughout the entirety of the game he will be the games punching bag, always the butt of the joke and needing to apologize for being a straight white male. The games goes on as expected, being an ideal experience for the target audience up until the ending/near the end where all those lovely characters previously mentioned get killed in the most brutal ways imaginable as well as insulting the target audience, deriding them for their beliefs and forcing all sorts of hard truths for them to listen to.
