They're not rare exceptions, most people who are interested enough in these games to install them probably want this. The only issue is does it actually deliver? Do the characters respond in a way that is good enough to pour time into rather than just having some eye candy. Pretty much your point is "Why not just fap to X?" Sometimes people want to do a little more than have a quick dry fap. Hell if all we wanted to do was fap we wouldn't even have porn games because they require too much input. Just removing censors wouldn't work. If the game had the unallowed parts able to be accessed through the game code even if you needed to mod the files to unlock it, is is part of the rating. That is why Hot Coffee gave GTA SA the AO rating, also why Oblivion got the M rating after it originally had the T rating.

I just use my imagination personally. You can set up scenes in Honey Select, I think – haven't "played" that game in particular but I have played similar. As well as costumes. TK17 has kinda shit graphics but that game let you set up entire scenes in locations of your choosing with costumes of your choosing and positions of your choosing, and all of those parts were modular. Obviously the girls were custom too. It can be a lot of effort, the interface was a heap of shit so I didn't really bother but I was interested in the concept I would hope Honey Select has similar capability.
