This question has been bugging me for some time. I recall a few episodes where alcohol was replicated even though it's supposedly banned on federation vessels. I also know there is an active drug trade at least in the Delta Quadrant and on that one planet full of Junkies in alpha.

So, if I were send through space time and ended up on the Enterprise and taken in as Picard's honored guest would I be able to replicate opioids from a few pills that were in my pocket? Would I be given special privileges to do this by citing my culture and forcing everyone to respect it?

The entire concept of synthehol always bothered me. It was supposed to replicate the "taste" of alcohol without the "effects" but alcohol doesn't HAVE a good taste; it's all the extra shit from the production of the various types of liquors that cause taste. Ethanol tastes like fucking paint thinner (basically unflavored moonshine) and nobody wants that. What people want in alcohol is the goddamned effects! The warmth, the comfort, the fuzziness, the slight loosening of social inhibitions. Without that, there is no point to drinking it at all.

That said, there's no reason the replicator can't replicate anything. In fact, your over-the-counter opioid pills are practically guaranteed to be partially synthetic (oxycodone, hydrocodone) or fully synthetic (fentanyl, methadone). Hardly anyone uses natural opium-derived products anymore. But if you asked the replicator to produce natural morphine as if it was scraped right out of a poppy, it could do so. It's just a collection of molecules.

perhaps those are specific "effects" deemed undesirable like euphoria and impairment. Synthehol then is perhaps a perferred way of imbibing a drug cocktail that gives the perfect recreational high. They could take them as a drop under the tongue or a hypospray, but where's the fun in that…?

As for opioids, captain obvious says there's obviously a reason why our heroes aren't perennially addicted to all manner of drugs and stimulations. One would assume they're either genetically engineered to be immune to disadvantageous dopamine stimulation or they closely manage their neurobiology.. possibly through nano-implants… and this is all done in such a routine and transparent way that they dont even remark on it.. will have to go back and watch that early TNG episode about the drug addicts. The device in The Game is strongly implied to be some sort of mind-control device primarily - like that Ferengi orb or sybok's hypnotism - and much more than merely a high addictive experience. Either way, it would have to short-circuit any kind of hedonic-management implant since we clearly see the crew experiencing euphoria and intense physical pleasure. Furthermore, we routinely are shown Starfleet officers inside and outside the crew who exhibit all manner of common human misery, low-grade depression, and various mildy-neurotic behaviors, so whatever this system is to mitigate/neutralize the risk of exposure to synthesized ultra-mega drugs.. it does so without going full comfortably numb tier or care-free enlightened being. like kirk told sybok, "i need my pain".

If I remember correctly:

in the episode with the drug addicted race there was a dialogue between Beverly and Wesley Crusher where they mention or at least imply, that humanity has developed beyond taking drugs. Basically the default Star Trek explanation.

And in "The Neutral Zone" Sonny tries to replicate drugs and discovers that only medical personal is allowed to do this. He then tries to get Dr. Crusher to give him some. Sonny also got real alcohol from the replicator in that episode.

I understand the the "green stuff" and the bloodwine doesn't apply. There are a few episodes where alcohol is replicated I just can't remember them now because it has been so long since I watched through the series. I do recall the body stealing dude on Voyage replicating "banned beverages" and I'm pretty sure it was alcohol considering he couldn't hold his shit together.

Apparently all replicators can produce real alcohol. No doubt the underlying technology is capable of producing any non-living substance; the firmware would limit its abilities so that some moron doesn't just replicate a few kilos of plutonium for shits and giggles. "Household replicators" are mentioned; so probably a good percentage of homes have them, assuming they are on an energy-rich comfy federation world, ship, or station. So how do you get your home replicator to replicate forbidden items? Well all it takes is one autistic chan-using Wesley Crusher type. Technology is unchained and so is knowledge… the only thing really stopping anyone is the practical hacking experience and perhaps the fizzy root beer good-goy federation citizen attitude.

I don't understand why they've put caffeine in some Root Beer but not some Ginger Ale or Lemon-Lime soft drinks. I want those fancy touch-screen soda fountains to dispense the caffeine separately and allow me to also add electrolytes or liquid vitamins as well.

Some root beer has caffeine for its bitter flavor. The ones that do, like Barq's, only have a small amount. IIRC, it's something like 6mg a serving. Nowhere near a regular soda.
