Self referential paradox anon. People in charge are the ones that report on what the problems were to the higher ups. Thus they will spin everything to their advantage. They will ignore that they butchered the script because of 'focus groups' when going against them was what put a property on the map in the first place and say it failed because of waning interest in the property. Overspending on big name actors will be left out but you had better be sure that paying for better animation will be selected as a reason for the budget. Only when the blame absolutely, positively can not be placed anywhere else do the people in charge of shitty decisions get what is coming to them. And even then, as long as the person was doing the accepted 'money making tactic' business says makes money they will face no real repercussions.

Thus, you have the reverse of Natural Selection. People of decent character that will admit fault and attempt to learn from their mistakes get stuck as mid level managers (the veterans responsible for running the show) while shit weasels that self promote and throw anyone and everyone else under the bus get promoted and make everyone's lives miserable.
