To be fair, they did subtly foreshadow it in episode 4 after Kotaro mentions the onsen. As for Lily's episode, it kind of ended up being a wash for me. Lily went from "cute loli that needs more character/backstory", to "cute trap with an effective backstory" but I don't much care for traps, so Lily stays where he is for me.
The episode also might've been better if they played Lily's death as a straight drama rather than making his death comedic by making him a trap and having the drama follow after.
