The answer lies in the games, where Mokou doesn't suddenly become weak or forget what is she doing after every spellcard death (each one brings her the great pain of dying then coming back to life). So we can conclude Mokou's body does not age in the typical sense but does remember somehow what was happening. Therefore she builds alcohol tolerance and hangover experience regardless of whether she dies drinking. This works only because experience or memory is not something hourai elixir affects (injury, disease, death only).
