So the latter represents the Japanese ideal of extreme difficulty, hard work, willingness to brutally main your enemy, and oddly complex characters in contrast to the Western ideal of literally being nice to opponents that try to kill you-doing so otherwise means you are worse than Satan, and death is just a slap on the wrist despite the game not being that diffcult?

Sans purposefully tries to fuck you up and break the rules because he knows that you are going to destroy the multiverse after you get through him. The healing items are there because Undertale is not really supposed to be a hard game in the first place, Sans and Undyne the Undying are the only parts of the game that genuinely attempt to be difficult.
