Hack/Ghost series from BlackBastard are Cave Story fangames with somewhat different gameplay.

Lethal Application by Daisessen is one of my freeware favorites, it looks different but it has the same colorful shooting action.

better designed levels made it a better game. cave story is fine but the design is very loose and not nearly as gratifying. the story and music are nicer but kero blaster has plenty of nice moments too.
