The best way to describe it is as a "single-player MMO". It's got a whole big storyline made of consecutive quests, with various multiplayer dungeons and boss fights spaced out in it. You do the story solo. While you could group up for stuff on the open maps, it's all made with the solo player in mind. This actually is really important, imho. Have your own first-person character killed , and you may feel frustrated at best. I think shit would be really spooky if you took this concept and intentionally played it out - for instance, if your actual player avatar sat in a room full of surveillance screens and had to guide another character through a maze filled with traps; the only way to actually control her being a two-way radio or another largely indirect tool.
Then imagine you could see shit she can't (such as the big demon manifesting behind her), and you probably would shout your lungs out as to tell her to GTFO of that room.

You're still missing the point. Prices can change, NPC locations can change, NPC pictures can change, things can be built and destroyed without emulating a single NPC. The only reason you need NPC's is if a player is seeing or otherwise interacting with them at that moment The further away the player is the larger the scale you can do your emulations on. When the player gets close you fire up some NPC's within his viewing range.
Take aging, if you want to get hyper realistic, and apparently he does: Instead of keeping an active NPC eating resources just set a timer and tie abilities and appearance to the timer. Only when and if that NPC is activated the timer is called and then the NPC is created with that modification.
