I strongly suspect that they simply fix up machine translation. Honestly it can be kind of difficult to tell apart scripts by someone who's doing that and someone who's just at a beginner level in Japanese, so I don't usually like to make that accusation. The biggest thing which makes me suspect it is that I've noticed little to no improvement in the translation quality since they've been doing it. Not to say that I often pay attention to what they do, but on occasion I'll take a quick look at what they post in the Yuzumori thread or if they happen to post something I liked in the TL thread. They still make plenty of basic errors, errors I wouldn't expect someone to make when they've been doing this so long.

That would be correct if the question was what the chance of picking the third imouto AND getting a red ribbon was. Basically an extra condition. But the problem was simply what the chance is for her picking a red ribbon. In simple cases like this you can also just write out all the possible events and then figure out the chance from there (with more options and such this is not really sensible anymore).
So since she has three imoutos and each imouto wears two ribbons you can write the events like this although in a case with more conditions and such a tree is a handier format (formatting probably will fuck up)
A,B,C imouto, r = red, b = blue
A -> r (1/6)
A -> r (1/6)
B -> b (1/6)
B -> b (1/6)
C -> r (1/6)
C -> b (1/6)
Each of those cases has a 1/6 chance and since each case is equally likely if the onee-sama chooses truly randomly and has no secret favorites you can add them up to get the chance of onee-sama ending up with a red ribbon, which is 1/2.

You're counting the wrong type of outcome. You need to count the number of red ribbons where the other ribbon is blue, and divide by the total number of red ribbons. You're counting the number of red ribbons and then dividing by the total number of ribbons.
