Not me personally but I know people who have.

Not sure about xbone but Sony stipulates in their contracts that PS4 dev kits need to be kept in a locked enclosure and are to never leave the building.

They're usually kept locked up, and the only one's that have surfaced have been from dev studios that went bankrupt and had their assets sold at auction

You can do console dev with the actual consoles + a PC with... well most likely nearly every console. The problem with emulators is that just because your code works in some asswipe's interpretation of what the hardware is doing, doesn't mean it will work on the real deal. For a real world example, run a classic nes series game on a gba emulator that doesn't simulate the arm's pipeline stages.
