The diver was probably supposed to just drown her , but due to her higher than average body fat ( seriously) she floated up pulling diver up into view. 2nd option. Injection she wouldnt feel due to water temp numbing legs.

I'm guessing she was coerced into approving the document, but had a conscience, and didn't feel right about it, and knew something was up.
