The problem with making suggestions is complex. But to simplify the issue - it really depends on the individual.
For example some people refuse to listen to a voice that is male so hypnotists like V1v3 or N1mj@ are instantly off the board even though they are pretty good at what they do.
Another issue for many people is the accent or dialect. Some hypnotists have obvious world locale origins that come out in their speaking voices. To use the example of V1v3 he's German and the voice is affected by that. Another example is the (real or put on) southern accent that tends to come out in Sh3ll3 R1v3rs files. There are many others but those are two quick examples.
Next issue is exactly what "theme" do you want in your files. Majority of the female hypnosis file makers are internet dommes so they tend to have themes of submission, humiliation, degradation, etc. heavy in their feminization files since that is what sells for them.
>TTS vs real voices?
This comes down to preference mainly. TTS some say have a better chance of bypassing your conscious mind, but only if well done. If not well done you hear glaringly obvious word defects or a mechanical sounding voice.
Real voices seem more personal and intimate, but you're liable to hear things that make the feeling be interrupted. Example would be MzD0m1n1c@ who would seemingly get bored in middle of her sessions and her voice will change out into a "God when will this stupid thing be over already" feeling or in a rush like she didn't think it was worth any more of her time than necessary to say the same words. Nothing quite breaks immersion like a hypnotist trying to lull a listener to sleep only to suddenly speed up the pace and rush out the message.

Again this is dependent upon the individual. It might feel like you're slightly off from reality or deeply immersed in it. It could feel like you're thoughts have slowed down or sped up. It perhaps will be like the world around you suddenly seems either brighter or dimmer. It may feel like your body is tingling or has become numb. It might make you feel cool or incredibly warm or even hot. It possibly will make you feel like the words are being said by your own subconcious or completely bypass your awareness entirely. It conceivably will be an experience that you remember or even by completely blanked out and only lingering feelings will remain after the fact.
There really is no right or wrong answer and is more about what works for you as an individual than what has worked for another John or Jane Smith.

Usually the files will tell you to try to relax and just settle into your chair or surface you're lying on. Attempting to focus on anything is usually counter productive. That being said sometimes a hypnotist will give you something to think about or an image or thought to focus on.
