It's because all of the bodies and faces are at about a 45 degree angle. They're facing diagonal to us, but their eyes are full-on sideways. It's like every character is uncannily off center. Usually in these kinds of visual novels, the characters' eyes face player.
The whole thing gives all of the characters a very unsettling feeling and it unifies their personalities into feeling the same, or as though they're related.

This fits better with an angsty teen than the joker, whether you think it's true or not. That wasn't really part of the Joker character except maybe a bit on the Killing Joke, but that's an stretch. It's not about realism, it's about "rules". As long as the story follows its own rules it doesn't have to be realistic. Fucking Star Wars isn't real, but that Han not shooting first, goes against Han's character. Bugs Bunny isn't realistic either, but he follows rules too: he never antagonizes first. Realistically speaking, Batman should have just increased police funding, while establishing more afterschool programs, but instead he's a psychopath in a fucking bat suit punching people in the jaw. But that's not his character. Character rules are pretty fucking important to a visual novel (Keyword: novel, as in story).
My problem is that it would go against Mia's character rules. Mia's established character is a virginal, meek, naive, and curious young girl that loves her parents, despite the fact they're fighting all the time. She already like the MC. She gives it up to the MC after he gets her parents back together. She has anal sex with the MC to preserve her virginity (in her own weird way, mirroring real teens with fundamentalist parents). You can say "muh story" all you want, but we both know this is fap material and forcing forks for the sake of "replayability" and shit is bass-ackwards. None of these storylines are fundamentally different in the way most romance routes are; for example, there's no reason you can't DP June as well as Mrs. Johnston, nor is there an intrinsic reason you can't have alone time with either of them.
