They can look up policy related shit with your social, even if you don't have any information on it with you. All you need is the policy name holder (your dad), the policy ID number and the group number (if it's group insurance which most company sponsored healthcare is). Just tell him to take a pic of the card and send it to you. You won't have a card specifically for you, it will just be the same as his.
If you don't have your own washer/dryer, it could also have been something weird related to what was washed in it before you. There's like a whole plethora of environmental shit it could be, though. Just sort of think to when you started noticing it and what new or changed things was around that. I know that's sort of vague and unhelpful but that's about all any Internal Medicine doctor will really do other than prescribe steroids for an allergic reaction and tell you to rewash and clean everything then come back in a few weeks if it didn't clear up.
