Killer self-replicating nanobots are theoretically possible, therefore they're inevitable. This is undesirable, but what's to stop someone from making 1? (1 is all it would takes)

They could be programmed to enter every person, go into their heart, replicate there until there's enough of them, then at a specified time coagulate together in the aortic valve shutting off bloodflow and killing every person at once. Or maybe 1 specific race or religion of person. Depending on the creator's intention.

Why does it need to be all at the same time? Better to make an airborne version of one of the most deadliest viruses and make it rely on some genetic trait that only the target population has. Sure eventually the target population will move somewhere the virus can't survive or something but not after 95% of them are dead, can just send in death squads for the last 5%.

Killing everyone at once makes it less likely that they find a cure. A bioweapon would kill people progressively.
