Personally, and speaking purely from fantasy, I'd make a distinction between punishments and "funishments". There'd be a set of serious rules which I'd expect her to follow at all times (unless the safeword was invoked) and a set of fun rules which she would be allowed to break. Breaking the fun rules would still earn her a punishment, but it would be more gentle and playful.
The real rules would be things like "always do your homework" and "always brush your teeth twice a day". The fun rules would be more like "no climbing on the furniture" or "always get permission before using the big-girl potty". The fun rules and their associated punishments would regularly change to suit our preferences.
Real punishments would primarily be boring and sometimes painful, such as corner time, writing lines, or a harsh spanking. Funishments would tend to involve more humiliation and milder pain, such as gentle spankings, potty restrictions, or taking her out for a walk while she tries to hold an enema.
If she broke the real rules I would make it clear it was a proper punishment and she wouldn't be getting out of it without using the safeword.
