Some do it because it's an interest for them for whatever reason. Entertaining and/or emotionally satisfying.

A large majority do it because it's either strictly sexual or sexual and emotionally satisfying.

I can't help but think in a way that this is what female ABDLs are like. Their content just seems so lifeless sometimes. "Rann's Doll" is a long winded comic that explore's the life of a cross dresser and has some real feels in it, but it also has some smut aspects to it. For a lot of people the AB/DL fetish is the same way, it's very emotional, but it's also very sexual. But Sammy is an autistic, furry, female, which seems to be the trio for not having any grasp of human emotions or behavior especially when it comes to this fetish. Like who the fuck puts a cloth diaper over a disposable? Why is Manda acting like a child and a grandmother at the same time all to avoid the reality of how a carer would act in this situation? Why is Star holding her tits as if she's not wearing a diaper and fucking on a paci? FFS, in the panel right above that Star is sitting all little and carefree trying to take off her shirt looking regressed and comfortable. If she must cover her breasts, could it be cute? It's why we sit here wondering what these comics are missing. Raan's Doll is similar to this story in that it embellishes in many ways. There are almost no crossdresser parties and meetups where everyone is still a guy yet they all look passing when dressed up (believe me, I looked for years for myself), the same way there are almost no massive diaper pool party meetups that are this where everyone is drinking diuretics. But the point is you make an interesting story from a fantastic situation. But it seems in order to ground this in real life, she is purposefully ignoring everything that would happen should such a situation happen in real life.

It's why I felt so uncomfortable reading through prior pages when Manda walked in on her and scolded her. The characters, in an effort to make them grounded in reality, overshot that point and ended up acting like how aliens would if they were portraying human situations. I couldn't tell what Manda was doing. Was she breaking immersion and actually pissed off, or was it part of the caretaker routine? If so, why was her mood swinging from one panel to the next? Is she a little or an adult, or both whenever it seems fit? And does Sammy actually enjoy this too, and to what extent? It's like watching an interaction of mixed messages.

And the smut comes with these mixed messages. I'm not sure if the story can be considered smut anymore. At first, it really looked like it. But after getting to the party, I can't tell if the meetup even has kinks or not. There are apparently kinky rooms, but why haven't we seen them? Will we ever? And if so, will everything be conveniently covered by a well placed leg or head as it's been? It's like I'm watching a story created by a schizophrenic.

It may sound counter intuitive but by not overstating the mess or the smell, it actually makes it a lot hotter.

I think the problem is that the story doesn't involve a typical AB Mary Sue. Rather Star is more like a "tourist" AB and the entire story, to me, is like a non-typical AB trying to adjust to a scene filled with typical ABs who are much more comfortable with it.

Like the whole thing with her wearing underwear with the diaper at first, I think to most typical ABs that's pretty weird, "going too slow" and so forth, but from the perspective of an outsider where being an AB isn't their primary motivating sexual desire it makes a lot more sense.

Comics and books are a unique medium in that such development can be slow and more realistic, where as in say a movie you have to cram everything into under 2 to 3 hours. She started out using colored lines, sort of an inverse Frank Asch sort of style, which gives the art a softer, more babyish sort of look… although it can be harder on the eyes.
